Sports & School Physicals In Gilbert, AZ
We can all agree that nothing is more important than the health of your child. While your child may look and feel great it’s still necessary to make sure that they are healthy enough for certain activities. If your child needs a sports physical form filled out, and they have had their well child exam within the last year, we can fill out their form based on that last well child visit. There is no need to schedule a separate visit! Just download the school sports form, fill out all the pages except the last page, and submit ALL the pages of the Physical form to our office for us to review and complete.
If they have not had a well child visit in the last year, schedule that and bring your Sports form with you (will all the pages filled out except the last) so that we can complete it at the visit.
What Goes Into a School Physical?
During your child’s visit there are a few things our pediatricians will need to go through with the parents or caregiver:
- Discuss your child’s medical history including immunizations, hospitalizations, surgeries, injuries, and medications they are taking
- Provide information about potential sports-related injuries and what to look for (e.g. concussions)
- Perform a thorough physical examination to check for potential health problems or past injuries that could cause issues in the future
- Complete the school physical forms, as provided by your child’s school (don’t forget to bring these forms with you to your child’s appointment)
